APPOINTMENTS ARE HIGHLY RECOMMENDED!  Please call the Assessor before you stop in to fill out your application!  Applications are due May 15th and require proof of income.

If you complete a tax return for the IRS, a copy IS REQUIRED to apply.  If you receive Social Security benefits, their annual 1099 form is also required.

Those who are under a doctor's care and unable to apply between Feb and May can request an extension to apply from the state with the Medical Extension Request Form below.
